Job Description

The Registered Nurse in Disability and Home Care Services is a pivotal member of our healthcare team, dedicated to delivering exceptional care to the aging population as well as individuals with disabilities. This professional is tasked with performing thorough assessments, managing health conditions, administering medication, and collaborating with a broad spectrum of healthcare specialists. The role is designed for a compassionate, skilled, and committed individual aiming to improve the health and daily living experiences of those under their care.

Main Responsibilities

  • Perform comprehensive health and risk assessments to gauge clients’ health needs both in the NDIS and home care environments.
  • Create, execute, and reassess personalised care plans that correspond with clients’ goals within the NDIS framework and home care requirements, fostering health and wellness.
  • Administer prescribed medications and treatments, vigilantly monitoring for any adverse effects or responses.
  • Work in tandem with an interdisciplinary team to ensure holistic care covering all health aspects.
  • Engage in audits and uphold standards and regulations pertinent to nursing practices within the NDIS and home care sectors.
  • Accurately document all evaluations, actions, and changes in conditions utilising electronic health record systems, ensuring documentation is current and precise.

Other Duties

  • Participate in meetings, training sessions, and continuing professional development activities as required.
  • Contribute to the development and review of policies and procedures related to nursing care within the context of the NDIS and home care services.
  • Assist with other tasks as directed by the management or relevant healthcare professional.

Requirements

  • Experience working within the disability and/or home care sector.
  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
    Degree in Nursing or equivalent.
  • Proficiency in using electronic health record systems.
  • Comprehensive knowledge of the unique functional needs and challenges faced by individuals with disabilities and the particular requirements of older adults in home care.
  • National Police Certificate (Mandatory)
  • Professional Qualifications: AHPHRA Certificate (Mandatory)
  • First Aid & CPR Certificate (Mandatory)
  • Driver’s license and access to a reliable vehicle (Mandatory)
