This is a fantastic opportunity for an experienced aged care senior support worker to take a step up into a coordinator role!  If you are looking for career progression in a flexible role then this is the role for you!

You will be motivated and proactive and keen to take on this newly created role and make it your own. Based in Norwest this casual role will see you initially working 8 hours per week on the days and times that suit you!  

Currently managing a small client base, this role will see you responsible for:

  • Coordination of care for all HCP clients
  • Preparation and management of all documentation
  • Rostering of suitably qualified support workers
  • Consultation with an RN for all clinical needs
  • Preparation of an individualised budget and ensuring services remain within budget
  • Monitoring client services on an ongoing basis
  • Management of client feedback 
  • Management of CMS ensuring regulatory and legislative compliance
  • General administrative tasks for both HCP clients and NDIS participants 

This is a role that you can truly make your own!  As the client base grows, there will be an opportunity for further hours or potentially a conversion to permanent part time employment.

To be considered for this role you will have:

  • Cert IV qualification in a relevant field of study 
  • Extensive experience in community based aged care
  • Current drivers licence and own reliable vehicle
  • Current Police Check 
  • NDIS Worker Screening Check (or willingness to obtain)
  • Excellent verbal and written communication skills
  • Sound computer skills with previous experience with a CMS held in high regard
