Job Description :

Complete Care Team has been providing quality in-home nursing and disability support for nearly 20 years.  We have recently established our Kings Park office and are seeking a Registered Nurse or Enrolled Nurse Service Coordinator to join our expanding team. In this role you'll be making a real difference in the lives of people with disability, supporting them to have the greatest sense of choice, control and confidence.

Your duties will include, though are not limited to:

  • Assisting and providing support to the branch Manager
  • Assisting RNs with developing person centred and clinical support plans
  • Developing and maintaining positive, respectful and supportive relationships with clients and the significant others in their lives
  • Developing and maintaining positive relationships with allied health professionals to promote positive outcomes for clients
  • Ensuring the safety and confidentiality of clients/ staff
  • Maintaining and updating staff/ client filing systems
  • Assisting with internal audits
  • Clinical support of all clients
  • assisting to maintain client rosters, and source and induct suitable staff
  • Providing ongoing support, mentoring and client-specific training to staff
  • Maintaining client records and monitoring funded support budgets as required
  • Participating in the ongoing improvement of Complete Care Team policies and procedures
  • Participating in an on-call after hours coordinator roster

Key Criteria

  • Completion of tertiary level relevant qualifications with demonstrated several years of experience in coordination and managing staff
  • Demonstration of effective communication skills with customers, staff and allied health professionals
  • Demonstrated experience in working directly with people with disability or Acquired Brain Injury
  • Ability to develop clinical support plans with person centred and strengths-based approaches
  • Proficiency in broad technology ability and a capacity to learn new technology and programs as required
  • Highly developed communication and problem-solving skills
  • Rostering skills highly desirable, and willingness to undertake occasional direct client shifts
  • Understanding culture and diversity
  • Hold a drivers licence & fully insured vehicle
  • Hold a National Police Check of no less than 1 year and a current Working with Children Check
  • Hold a current First Aid Certificate
  • Mental Health First Aid is desirable


Salary packaging available for successful applicant

This is a casual position (35-40 hours a week)
