Job Description :

Complete Care Team has been providing quality in-home nursing and disability support for nearly 20 years.  We have recently established our Kings Park office and are seeking a Service Coordinator to join our expanding team.

In this role you'll be making a real difference in the lives of people with living with disability and Traumatic Brain Injury, supporting them to have the greatest sense of choice, control and confidence through:

  • developing and maintaining positive, respectful and supportive relationships with the participant and their significant others;
  • fostering strong relationships with case managers and other allied health professionals to assist in achieving the best outcomes for the participant;
  • developing person centered and clinical support plans that focus on the participants needs, strengths and individual goals.

In this role you will be also responsible for:

  • providing supervision and education to field staff to ensure a skilled, competent workforce;
  • maintaining client records and monitoring funded support budgets as required;
  • participating in the ongoing improvement of Complete Care Team's policies and procedures;
  • assisting with rostering and a shared on-call after hours coordinator roster as required.

Completion of relevant tertiary level qualifications in Social Work/ Human Services such as RNEN, OT, Social Worker or relevant discipline


  • Demonstrated experience in working directly with people with disability or Acquired Brain Injury
  • Ability to develop clinical support plans with person centered and strengths-based approaches
  • Demonstrated experience in coordination and managing staff
  • Rostering skills highly desirable, as well as a willingness to undertake occasional direct client shifts
  • Strong communication skill
  • Advanced computer literacy 


  • Drivers licence & fully insured vehicle
  • National Police Clearance (valid within 6 months)
  • Working With Children Check
  • First Aid


  • 12 month contract with view to permanency
